Requests for Medical Records

Hawthorn Medical has partnered with Sharecare for patient medical record requests.

**Note: If you are a provider/health facility faxing records to Hawthorn Medical, call 508-996-3991. You will be given the correct fax number for the department that should receive your records.

Print the Authorization to Use and Disclose Protected Health Information.
You may drop the form in the lockbox at 535 Faunce Corner Road, Dartmouth – 1st floor Reception area.
You may also call Sharecare Customer Service at 508-718-2010 for assistance.

To request Imaging/Radiology results on a CD, call the Diagnostic Imaging Department at 508-961-0893 or go to 531 Faunce Corner Road, Dartmouth, first floor. To prepare the imaging for CD transfer, we ask for at least 24 hours notice (unless it is an urgent situation).
