Important Information about your Medical Record and your Patient Portal
On Sunday, August 24, Hawthorn Medical moved to a licensed Epic electronic medical record (EMR) system for access to Brown University Health’s LifeChart® and the patient portal called MyChart.
We strongly encourage you to sign up for the MyChart patient portal to allow for the best patient experience. The MyChart portal is where you will view new health information beginning August 24.
You have options for signing up. You can set up an account at www.brownhealth.org/mychart and follow the steps. If you sign up before a visit where an activation code would be provided, choose the “No Activation Code” option. You will need to verify your identity through Experian, a consumer credit reporting company.
At your first visit after August 24, you will be offered the chance to sign up then or receive a link to complete the process. An activation code will be given to you.
If you are having trouble finding Brown Health Medical Group as an option, set the location to Rhode Island.

PRE-REGISTRATION AVAILABLE
The move to the new electronic medical record has resulted in the registration staff having to re-enter all patient information, scan ID and insurance cards, and obtain signed consent forms. This requires more time to fully register our patients when they come for an appointment. PLEASE BRING YOUR PHOTO ID AND INSURANCE CARDS WITH YOU TO YOUR VISIT.
A pre-registration option is available in the MyChart portal where you can update your demographic and insurance information and sign consent forms that will shorten the time it takes for you to check-in. If you have a MyChart account and selected email or text as your communication preference, you will receive a message one day prior to your appointment so that you can pre-register.
In MyChart, go to Pre Check-in on home page or within visit details and follow the steps. You will still need to stop at registration to be checked in on the day of your appointment.
Important Information about Changes to the current Steward/Revere Health patient portal that you should be aware of:
The Steward/Revere Health portal is now Read-Only. No new health information will be added to this portal (labs results, appointments, etc.). We are unsure how long the read-only access will be available. At some point, Revere Health will permanently disable access. The information from RevereConnect will not be moved to the new portal. However, the information in your medical record will move to Epic.

Due to the complexity of this transition and the many features the EMR offers, over the next several months the Hawthorn team will be continuing to learn all the different ways they can best serve patients using this new platform. We appreciate your patience and understanding as our staff becomes more familiar with Epic and the new patient portal.
